Health Form

Filling in the Health Form:

  1. Download the Health Form here or by going to the downloads section.
  2. All the immunity proofs; except the MRSA, (copies of the test results and not a copy of the vaccination card) can be sent prior to your commencement of elective period, along with the health form signed by a competent authority.
  3. When the immunity proofs are ready (for e.g. Varicella, TB), we ask you to kindly email copy of the results to the undersigned.
  4. The MRSA test is to be taken not earlier than 12 weeks prior to arrival and the result should be scanned and sent by e-mail. Kindly send the document in jpeg or pdf format.
  5. Once the Health form is received the Department of Health assesses received Health Forms. Should the doctor note the need of any updates, you will be in time to be contacted in order to update your records.  Then the MRSA test result is to be scanned once it is within the time frame.
  6. Please note that these Health Forms are approved by the Department of Health. Without the approval of the Health Department, students will not be allowed on the wards.

Note: Incomplete health forms will not be accepted. Health forms are to be in English only and received not earlier than 12 weeks and not later than 6 weeks from the start of the elective programme.
