a. Download Health Form by clicking here or by going to the downloads section. Health forms are to be in English only and received not earlier than 8 weeks and not later than 4 weeks.

b. Send the COMPLETE Health Form by post to the following address:

Since the MRSA test result might take sometime to be issued, students are to send the rest of the completed health form according to the indicated time frame. Once the MRSA test result is at hand, a copy is to be sent via email on edocuments@mmsa.org.mt. A hard copy is also to be sent via post at the address indicated above.

Note: Incomplete health forms, not in English or not within the above time frame will not be considered. Email the NEO.NORE on neo.nore@mmsa.org.mt if needing more information regarding the Health Form.
